SOWING SEEDS COUNSELING LLC BILLING POLICIES AND PRICING
EFFECTIVE DATE OF THIS NOTICE: This notice went into effect on February 28, 2023; updated December 24, 2025.
We (“We, Our, Us” referring to Sowing Seeds Counseling LLC)
(“You, Your, Patient” referring to current or past patient)
Insurance Coverage & Deductibles:
- Patient Responsibility: Patients are solely responsible for verifying their mental health insurance coverage, including deductibles, co-pays, and out-of-pocket maximums.
- Insurance Verification: Patients are encouraged to contact their insurance company directly to understand their coverage and benefits.
- Claims Submission: Patients are responsible for providing accurate insurance information and necessary authorizations.
Payment for Services:
Standard Fee: $150 per hour of counseling services.
Cancellation Fee: Patients are required to give 24-hours’ notice for cancellation of an appointment or pay a cancellation fee of $50.
- Payment Due: Payment for counseling services rendered is expected in full within 30 days of the service date.
- Late Payments: Late payments may be subject to lates fee or forwarded to a third-party collection agency.
Sliding Fee Scale:
- Eligibility: A sliding fee scale is available to patients who qualify:
- Documentation: To be considered for the sliding fee scale, patient: are required to provide documentation of their financial situation (e.g., income statements, proof of unemployment).
- Sliding Fee Calculation: A sliding fee calculation system will be used to determine a fair price for services.
Good Faith Estimate:
The calculated fair price will be used in the good Faith Estimate statement provided to patients as applicable and required by Federal Law.
Subpoena for Testimony:
- Fees: The fee for a subpoena for testimony is $750 per day and includes time spent preparing documents and travel.
- Court Order: Subpoenas for testimony are only valid with a letter from the Court.
